By booking or receiving services from The Home Practice LLC ("The Home Practice," "we," "us"), you ("Client") acknowledge and agree to the following:
Assumption of Risk. Services provided by The Home Practice — including but not limited to assisted stretching, massage therapy, personal training, yoga, chiropractic care, and physical therapy — carry inherent risks, including but not limited to muscle soreness, strain, bruising, or aggravation of existing conditions. You voluntarily assume all such risks.
Health Disclosure. You represent that you have disclosed any medical conditions, injuries, pregnancy, or physical limitations relevant to your session, and that you are physically fit to participate. Services provided are not a substitute for medical diagnosis or treatment.
Release of Liability. To the fullest extent permitted by law, you release, waive, and discharge The Home Practice LLC, its owners, employees, contractors, and practitioners from any and all liability, claims, or damages arising from your receipt of services, except in cases of gross negligence or willful misconduct.
In-Home Service Acknowledgment. Because services are performed in your home, you agree to provide a safe, reasonably clean space for the session and to disclose any household hazards (pets, uneven flooring, etc.) in advance.
Right to Refuse Service. The Home Practice reserves the right to decline or discontinue service if a session presents a safety concern for the client or practitioner.
Governing Law. This waiver is governed by the laws of the State of Colorado.
